Cambria Primary Care are working together to provide the following services together.
Teledermatology and Primary Care Dermatology Service
Teledermatogy Service – The practices have access to a teledermatology service provided by consultant dermatologist in the University Hospital of Wales in Cardiff. For the majority of patients requiring a consultant opinion on a skin condition the GP is able to take a photograph and send it to Cardiff. The consultant provides advice on the diagnosis and treatment directly back to the GP reducing the need for the patient to travel to Carmarthen where the nearest consultant dermatologist is based.
Primary Care Dermatology Service – The GPs from all four practices are able to refer directly to the GP with special interest dermatology clinic based at Borth Surgery. The expert GP is able to diagnose and treat a number of conditions and also perform minor surgery again reducing the need for the patient to travel to Carmarthen.
Care Home Enhanced Service
The four GP practices are working together to look after their patients who are living in care homes to ensure that they have a regular review by their together GP with a frailty nurse and pharmacist.
Physiotherapy Service
The four GP practices now have appointments available for Physiotherapy without needing to see the GP. Once an appointment has been made the Physiotherapist will:
- Assess you and diagnose what's happening
- Give expert advice on how to manage your condition
- Refer you on to specialist services if necessary
Simply call in at your GP Surgery and ask reception for more information. Clinics will be based in Padarn Surgery in the North and Tanyfron Surgery in the South.
Back Office
The four GP practices are working together to standardise the way that the clinical letters are managed when they are received by the surgeries. Once completed this will ease workload on clinicians.
Social Prescribing
The four GP practices are working with Rural Health and Care Wales to pilot a referral service for patients for social prescribing. It is hoped to develop this service further.
